Operation Update Form

Notarized Statement of Transfer from seller

Legal Ad 2-3 weeks

Floor Plan (signed and dated) On no larger than 11” x 17” paper

Fire Alarm Test Report (any business with a full alarm system) must be current

Inspection (unless the inspection has been done within the past six months)

Licensing Board Hearing (licensee IS required to be present at meeting)
