Position Opening
The Town of Provincetown is accepting applications for the position of
Council on Aging Outreach Coordinator.
Responsibilities include outreach to town residents 60+ years old and
residents under 60 with disabilities and their families to provide information
and referral, advocacy and support services; intake assessments to identify
issues, evaluate needs, develop and implement plan with client and family via
home or office visits; and case management, including follow-up. Position reports to Director of Council on
Aging, Public Health and Human Services.
College degree in social work or human services field preferred. Requirements include: minimum of five years
social services experience in providing direct services to clients, preferably
elderly population; knowledge of local, state and federal support services; automobile
for home visits and meetings; computer literacy and competency skills.
