Dear Applicant:

Enclosed is the information you requested for the one-bedroom rental at 27A Nelson Avenue in Provincetown. The rent for that unit is $986 per month, NOT including utilities. Qualified applicants will be contacted when the unit is ready for viewing.

Please check the Rent/Income Table to be sure that your household income is within the guidelines. Household income includes earned and unearned income received by members of your household who are 18 and older. Income earned by full-time students is excluded.

Be sure to read the directions for completing the application very carefully! The application is double-sided. Complete both sides of all pages. Do not leave blanks. If not applicable, write “n/a” or “0”. Incomplete applications or those that do not include all necessary documentation will not be processed.

Application screening will include a personal interview, credit check, criminal history check, landlord references and verification of income/ability to pay rent. All household members over the age of 18 must sign the release form permitting Lower Cape Cod CDC to do a credit, eviction and criminal history check.

DEADLINE: Applications and all supporting documents must be postmarked by 4:00 pm on Monday, March 10, 2008. Completed signed original applications may either be mailed or dropped off in person at the Lower Cape Cod CDC office in Eastham. Faxed or emailed applications will not be accepted. Applications will be processed on a first come first served basis.
